The CPT code for tympanocentesis is 69420.LT for the left ear and 69420.RT for the right ear. A bilateral tap is coded 69420.50. Medicaid reimbursement for the procedure averages $80 in addition to the office visit fee, but can vary measurably from state to state. In Maryland, the current Medicaid fee schedule provides less than $20 reimbursement for a unilateral tap. In Alaska, the same reimbursement is $186. Reimbursement for tympanocentesis performed on private insurance or managed care patients is higher, ranging from $95 to over $180 for one ear (not including the office visit fee).
