Indications for tympanocentesis:

For immediate relief of pain, pressure, or hearing loss associated with a middle ear effusion.

For aspiration of middle ear fluid for culture analysis.

For therapeutic drainage of middle ear effusion associated with acute otitis media.

When acute otitis media fails to respond to appropriate antibiotic therapy.  

For AOM patients with multiple antibiotic allergies.

As an adjunct to the observation option, tympanocentesis relieves pain and symptoms while providing aspirate for culture.  This enables precisely targeted antibiotic therapy for patients who remain symptomatic after the observation period.

Contraindications for tympanocentesis:

Known anomalous position of the jugular bulb

Uncooperative patient
